Creating a Questionnaire
First off, before you add a Questionnaire to a page on your website, you must create a Questionnaire. You can do this by entering into the "Modules" tab beside the "Site Map" option at the top of your page. On the left side of the page scroll down until you see "Manage Questionnaires" and click on it. This will bring up the Questionnaire Options panel at the top right corner of your page.

By choosing this option, you will be shown all of the Questionnaires that have been created and are currently available.
Edit Questionnaire
Some exciting features based on the following questionnaire options:
Name
Time Limit Minutes
Display Count-down clock: Yes No
Auto Save response data after ever page: Yes No
Enable Progress Bar: Yes No Progress Bar Advanced Controls Progress Bar Color, Size: Send a copy to the user: Yes No Send Copy From (Email Address): Subject: Email Message:
Delete Questionnaire
This will allow you to delete whichever Questionnaire you wish.
Manage Questionnaire
This is where the fun begins and you have the opportunity to begin Adding the Questions to your Questionnaire.

Add Questionnaire
You will be able to create a new Questionnaire.
Copy Items
Allows you to copy the questions from one questionnaire to another.
Inserting a Questionnaire onto a Page

Type of Page Drop Down
Once you have created a Questionnaire, create a new Section, Category or Page within the Site Map portion of SiteApex depending on where you want the Questionnaire to go on your website. In the "Type of Page" drop down box, select Questionnaire and click "Submit" at the bottom of the page.
Selecting a Questionnaire
It will now prompt you to select a Questionnaire you wish to add. Select the Questionnaire you desire on the page from the dropdown list and click "Submit" once more. |

Publish Site and you now have a fully operational Questionnaire on your website!
